Unified Permitting Form is a form to eliminate the duplication of some hazardous substances in different lists. It contains data about the handler (import or export) of hazardous substances, a complete description of the hazardous substances intended to be handled periodically and a description of disposal. In addition to the available means of transport (by land-rail-sea-air-internal waterways) and their routings and schedules as well as a declaration of the veracity of data stated in such document.

نموذج الإفراج الجمركي الموحد هو نموذج موحد لمنع الازدواجية و التضـارب و تسهيل إجراءات الإفراج الجمركي للمواد الخطرة عبر المنافذ الجمركية في القوائم المختلفة للمواد الخطرة كما تحتوي علي بيانات خاصة بالقائم بالمواد الخطرة و كذلك علي توصيف كامل للمواد الخطرة المزمع تداولها باستمرار ووصف كامل لعملية التخلص . توصيف وسائل النقل المتوخاة ( بري – سكة حديد  بحري – جو – مياه داخلية ) و تحديد خطوط سيرها و مراقبتها و اخيرا إقرار لصحة البيانات الواردة في هذه الوثيقة
